A significant global clinical trial, co-led by McMaster University, has revealed that fluvoxamine, an inexpensive and widely accessible antidepressant, can offer meaningful relief for adults grappling with this pervasive symptom. Despite the immense global burden of long COVID, which is estimated to affect approximately 65 million people worldwide, evidence-based treatment options remain remarkably scarce.

The REVIVE-TOGETHER trial, a large-scale, randomized, placebo-controlled study, sought to address this critical unmet need. Researchers investigated the efficacy of fluvoxamine (marketed as Luvox), a selective serotonin reuptake inhibitor (SSRI) already established in the treatment of depression, obsessive-compulsive disorder, and other mental health conditions. "Fluvoxamine showed consistent and meaningful benefits, and because it’s already widely used and well understood, it has clear potential for clinical use."

The REVIVE-TOGETHER Trial: A Global Collaboration

The collaborative effort behind the REVIVE-TOGETHER trial involved researchers from Canada, Brazil, and the United States. Clinical sites were strategically established in Belo Horizonte and throughout the Minas Gerais region of Brazil, a location chosen for its high prevalence of COVID-19 cases and established research infrastructure. The international consortium included investigators from renowned institutions such as McMaster University, the University of British Columbia, Stanford University, the University of Pittsburgh, Duke University, Georgetown University, and several leading Brazilian academic and research centers.

A Rigorous Approach to Testing Existing Therapies

The trial design incorporated 399 adults residing in Brazil who had experienced persistent fatigue for at least 90 days following a confirmed SARS-CoV-2 infection. Participants were randomly assigned to one of three treatment arms: receiving fluvoxamine, metformin, or a placebo. The treatment period lasted for 60 days.

"We wanted to test whether two existing, widely available, and affordable medications could help," explained Professor Mills. "Both had biological reasons to think they might work against long COVID fatigue, but neither had been rigorously tested for this purpose in a proper clinical trial."

The results demonstrated a clear advantage for fluvoxamine. Statistical analysis indicated a 99 percent probability that the medication was more effective than the placebo in reducing fatigue levels. Beyond symptom improvement, participants who received fluvoxamine also reported significant gains in their overall quality of life, as measured by several established assessment tools.

In contrast, metformin did not yield comparable results in this specific context. While prior research had suggested a potential benefit of metformin in reducing the risk of developing long COVID when administered during the initial stages of infection, this trial found no meaningful improvement for individuals already experiencing established long COVID fatigue. This distinction highlights the importance of testing interventions in the specific patient populations and disease stages for which they are intended.

Innovative Trial Design: Accelerating Discovery

A key aspect of the REVIVE-TOGETHER trial was its utilization of a Bayesian adaptive trial design. This sophisticated methodology allows researchers to dynamically adjust the trial based on accumulating data, enabling them to stop individual treatment groups early if the evidence becomes sufficiently clear. Gilmar Reis, the lead author of the study and a researcher with Cardresearch, a Brazilian clinical research center based in Belo Horizonte, emphasized the innovative nature of the trial design. "The trial used a sophisticated adaptive design that allowed it to reach conclusions more efficiently than traditional trials, stopping early when the evidence was clear enough – a design innovation as important as the findings themselves," Reis commented. Reis also holds a part-time associate professor position at McMaster University.
